Terros Health is pleased to share an exciting and challenging opportunity for a Children's Wellness Coach position, based out of locations in Phoenix, AZ, Glendale, AZ, and Chandler, AZ. This is a direct service position working in collaboration with the Boys and Girls Clubs of the Valley at assigned sites. This position will interact directly with the Children and Staff and Families at the Clubs and use the designated prevention model Positive Action while also implementing other clinical approaches based on Club needs. This team member needs to have a passion for working with Children in an afterschool setting, be willing to be an active participant/lead in therapeutic activities, and act as a liaison for Terros Health and these co-locations. Responsibilities









Seeking a Children's Wellness Coach for a brand new, exciting collaboration with the Boys and Girls Clubs of the Valley at assigned BGCAZ locations. This position will interact directly with the Children and Staff and Families at the Clubs and use the designated prevention model Positive Action while also implementing other clinical approaches based on Club needs. Full-Time Employed: 40 hours/weekM-F: 11a-7pLocation: co-located within ONE Boys and Girls Clubs of the Valley location (listed below):Compadres Branch - 300 E. Chandler Blvd, Chandler, AZ 85225Swift Branch - 6420 W.   • Conducts the engagement process with families to identify natural/formal supports as outlined in the Arizona Model, Child and Family Team (CFT) process, and Regional Behavioral Health Authority (RBHA) for enrolled families.
  • Facilitates CFT/Service Plan meetings according to the Arizona Model and RBHA guidelines in order to create individualized service plans as identified by the CFT members.
  • Ensures families understand the behavioral health process, the role of CFT Facilitator, and ensure family participation that links them to natural or community-based supports.
  • Coordinates and schedules CFT/Service Plan meetings with natural, formal supports, and system stakeholders
  • Submits appropriate referrals to clinical outpatient services within agency and other behavioral health providers identified by the CFT.
  • Updates and maintains electronic medical records on each case which include but not limited to; Annual Assessment Update, RBHA demographics, Child and adolescent service intensity instrument (CALOCUS), and CFT/Service Plan
  • Completes strength assessments and crisis/safety plans with families according to RBHA and the Arizona Department of Health Services (ADHS) guidelines.
  • Develops positive supportive relationships with consumers, family, stakeholders, and other Terros employees and Boys and Girls Club employees. Actively leads group prevention or therapeutic group activities at assigned Boys and Girls Clubs of the Valley locations.*This position is funded by a grant, which is for a pre-determined timeframe of 3 years.     Qualifications









    Education/Experience:
    • GED/High school diploma with a minimum of 1.5 years of behavioral health experience or,An associate's degree in the field of medical or behavioral health, orA bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, orA bachelor's degree in the field of behavioral, orA master's degree or higher in the field of behavioral health
    • Preferred - Bilingual proficiency in English and Spanish.Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferredValid AZ Driver License. Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health's driving policyMust have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employmentInterpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community membersGood communication skills, evidenced by a non-judgmental approach, effective listening and positive interactionsBasic understanding of the DSM-V preferredCertified Case Manager strongly preferredMust have or be able to attain CPR/First Aid certification within 60 days of assuming roleMust pass background check, TB test and other pre-employment screeningPhysical demands of this position are representative of those that must be met by an employee to successfully perform the essential functions of this job.
