part of Optum, has an immediate opening for a friendly, patient
focused and detail oriented Associate Pharmacy Care Coordinator to
join our team. Hours: Monday - Friday, 8:30am to 5pm Location:
Phoenix, AZ Primary Responsibilities:Responsible for maintaining
and restocking medications and supplies for providers in assigned
territoryOrganize and maintain a consistent stock of inventory by
ordering and receiving stockEnsure accuracy of the digital vs the
physical inventory on handEnsure provider compliance by tracking
medication and supply usage and documentationMaintain medical
refrigerator, document daily temperatures, and download temperature
reports. Provide support to the market during annual flu
campaignProvide support to the pharmacist and market pharmacy team
for clinical pharmacy programs by completing phone calls, faxing
patients, providers, pharmacies and/or health plansCall and follow
up with patients as directed by the pharmacist and will relay
findings to pharmacistContact and follow-up with community
providers and prescribers via fax and phone regarding pharmacist
recommendations, track status, triage pharmacy fax inbox for
responsesContact community pharmacies to verify co-pays and/or to
investigate medication access issues on behalf of patientsEducates
patients on pharmacy-based rules related to medication resources or
assistance programProvide support and guidance to patients and the
market on medication adherence by actively communicating pill
packaging, home delivery, and mail order solutionsExecute and
maintains reports and mail patient education to patientsTriage
incoming Landmark Pharmacy tasks and referrals and track
statusPerform other duties and projects as assigned What are the

in. Required Qualifications:High School Diploma/GED (or higher) OR
2+ years of pharmacy or PBM experience1+ year of pharmacy or PBM
experienceIntermediate level of proficiency with Microsoft Office
(including Excel) Preferred Qualifications:Active/current National
Pharmacy Technician Certification (PTCB, NCCT, or ExCPT)3+ years of
current or recent pharmacy or PBM experienceGeriatric or Medicare
Part D experience Soft Skills:Ability to work in a fast-paced
environment that is constantly changingExcellent telephonic
