step of the way. Join Our Team: We are seeking experienced and
empathetic Hospice RN Case Managers to join our dedicated
Scottsdale team. In this role, you will be essential in delivering
high-quality hospice care and guiding patients and families through
their journey with dignity and compassion. Positions Available: 2
Full-Time RN Case Managers 1 covering the West Valley 1 covering
the East Valley Key Responsibilities: Manage and coordinate care
for a caseload of hospice patients. Develop, implement, and update
individualized care plans. Provide skilled nursing visits,
including symptom management and end-of-life care. Educate
patients, families, and caregivers on hospice philosophy and care
expectations. Collaborate with an interdisciplinary team, including
physicians, social workers, chaplains, and aides. Ensure accurate,
timely documentation according to regulatory and company standards.
Participate in on-call rotation as needed. Qualifications: Current,
unrestricted RN license in Arizona. Minimum of 1 year hospice
experience preferred. Strong assessment, critical thinking, and
communication skills. Compassionate, empathetic, and committed to
high-quality end-of-life care. Reliable transportation, valid
