level of care. General Position Summary: As a Social Worker, you
will be instrumental in providing psychosocial and crisis
intervention consultations to patients in various hospital
departments. Your role includes conducting psychosocial
assessments, tackling issues around abuse, neglect, chemical
dependency, behavioral health, domestic violence, suicide attempts,
traumatic events, dysfunctional relationships, and end of life
matters. You will manage individual and at-risk patients to achieve
optimal clinical, financial, operational, and satisfaction
outcomes, identifying potential barriers to patient discharge and
facilitating their resolution. As a patient and family advocate,
you will guide them to actively participate in their care and
navigate the healthcare system. Essential Functions / Major
Responsibilities: - Deliver psychosocial and crisis intervention
consultations in various inpatient and outpatient departments. -
Assess patients and families to identify psychosocial needs and
discharge requirements. - Develop a comprehensive plan in
collaboration with patients, families, the interdisciplinary team,
and external agencies to meet these needs. - Implement a safe
discharge plan utilizing appropriate and available resources. -
Engage in community-based functions and provide assistance to
community members with social service needs. - Participate in daily
Interdisciplinary Team meetings to develop and manage the care
transition patient discharge plan effectively and communicate the
plan across the continuum of care. - Coordinate healthcare services
and identify issues that may delay patient discharge, facilitating
their resolution. - Advocate for patients and their families,
engaging them as active participants in their care and assisting
them in navigating the healthcare system. - Assess, plan,
implement, coordinate, monitor, and evaluate appropriate patient
disposition in collaboration with the healthcare team to formulate
and achieve a cohesive, comprehensive discharge plan. - Facilitate
and participate in process improvement activities for patient
populations to achieve optimal outcomes. - Establish and promote
collaborative relationships with physicians, payers, and other
members of the healthcare team. - Provide education, information,
direction, and support to patients, families, caregivers, and
multidisciplinary healthcare team members. - Stay updated with all
regulatory guidelines, social work, and case management standards.
Participate in internal/external continuing education and quality
improvement activities. - Display proper etiquette and mannerisms
that reflect the Shine Behavior Standards. - Uphold the Patient
Safety Standards as a core value of the organization. Secondary
Functions: - Participate in departmental and association-wide
meetings, forums, and seminars. - Review department and
association-wide policies and procedures annually. - Perform other
duties as assigned. Job Scope: This role involves frequent new and
varied work situations, a high degree of complexity, typical
operation from established procedures, and may contribute to the
development of new services, programs, or processes. The
performance is under minimal to moderate supervision. Interpersonal
Contacts: You will interact with others both inside and outside the
association, across departments and locations, including staff,
patients, and physicians. The role often involves
confidential/sensitive information necessitating discretion at all
times. Contacts are made via telephone, e-mail, and face-to-face
interaction. Specific Job Skills & Mental Activities: This position
requires a deep understanding of crisis intervention, mediation,
counseling, behavioral health legalities, and psychopharmacology.
Operational knowledge of department equipment and common computer
programs is required. The employee must be service-oriented, have
excellent customer service skills, organizational skills,
multitasking skills, professional interpersonal skills, time
management skills, public speaking skills, critical thinking
skills, and telephone etiquette. Proficiency in English reading,
writing, speaking, and understanding is necessary. Education and/or
Experience: - Master’s Degree in social work from an accredited
school (preferred). - Basic computer skills (required). - BLS/CPR
certification (preferred upon hire; required within 30 days of hire
and must be maintained throughout employment). - AZ finger print
clearance card (Application to be completed within 14 days of hire
date; required). - One year of related experience in crisis
intervention and/or hospital social work (preferred). - Licensure
(LMSW) in Social Work within 1 year of hire (required). Physical
Demands & Job Conditions: This job requires light physical
activity, including exerting up to 20 lbs. of force occasionally,
and/or up to 10 lbs. of force frequently. The position involves
extensive close work, computer work, and interaction with upset
individuals. Activities include finger dexterity, standing,
walking, stooping, talking, reaching, feeling, sitting, bending,
kneeling, grasping, listening/hearing, handling, lifting up to 10
pounds, and repetitive motions of the hands, wrists, and feet. OSHA
Exposure Category: The role is classified under OSHA Exposure
Category 3, which involves no regular exposure to blood, body
fluids, or tissues. Please note that this is a safety-sensitive
position, and requires adherence to our organization's Patient
Safety Standards and Shine Behavior Standards. Qualifications:
Required Education: - Masters or better in Social Work. Required
